A1: Get or search for a Standard Form 180 - REQUEST PERTAINING TO MILITARY RECORDS and in Section 2 - Purpose: check the MEDICAL box. Complete to form and in Section III, make sure you give the VA the correct address where to mail the results to you.
Then send it to the correct location depending on your CURRENT STATUS OF SERVICE MEMBER on last page of form. This might take a long time depending how far back your records go and they will send you a CD. Good Luck. (JL) 12/25/20
