Q611:  My wife has power of attorney over all my affairs. She waits on me to help me dress and many other things pertaining to my disabilities. I can't remember much except Vietnam.  My wife takes me to doctors, etc.  They amputated part of my left foot for the virus we caught in Nam.  If there's a form to fill out to apply for SMC please inform me of the number? Thank you.

A1:  Contact the DAV in your area for assistance.  (BF)  12/20/20

     

A2:  U can file for home care.  Also there may be other conditions that you can file for almost everything can be done on line today.  If not get a DAV or VA representative to file for u.  Also u get payed from the day u filed.  (SG)  12/20/20

     

A3:  You need to have a doctor to fill out Aid and Attendance rom. Make sure she has filled out a 21-22 power of attorney for the VA .  (GR)  12/20/20

     

A4:  Have your wife apply for caregiver. She can go online and fill it out and submit her form or print it and mail it. They will contact her and interview you and her both to determine eligibility. There are different levels of compensation she could be eligible for. It's an easy interview, I did it with my Vietnam veteran husband. A5:  One thing I might point out is that VA does NOT recognize civilian power of attorneys. If you would like for your wife to be able to speak to the VA for you, complete a VA Form 21-0845 and submit it to the VA. Then when the VA calls, or you want to ask VA a question, she can speak to the VA for you. If you are unable to manage your finances based on medical evidence, then the VA will develop for a "fiduciary". After this process is complete, and your wife, for example, is assigned as your fiduciary, then she will be the one to sign all your VA forms and handle all your matters with the VA for you.  As far as filing for SMC, the only form to file for SMC is the VA Form 21-2680. This is the form you use when you are asking for the "Homebound" or "Aid & Attendance" SMC. If your partial loss of you foot is related to a service connected disability, then the VA will automatically develop for SMCs.  (DS)  12/21/20
