Q236:  My original military/medical records were lost in a flood. I contacted the necessary services to get a copy with no success. How can I get a copy of my updated DD214? I do not have Page 4 which is very needed. Only have DD214 reflecting when I first went into the Air Force. Please help! Thank you for your assistance and service.

Use Form Below To Respond

A1:  You should be able to obtain the appropriate DD-214 documents from archives gov. That and other documents.  (KD)  9/10/19

   

A2:  Go to an Accredited VSO with a Service Organization (Am Legion, VFW, DAV) and have him/her submit a request through your nearest Congressional Office. Attach a SF180 for the periods of service an ask for your OMPF (Official Military Personnel Files). The SF180 has a block for the DD-214 request.  (GH)  9/10/19

   

A3:  If you go on line go to va.gov. Go down their page and on the right side is a link to request a DD214. It can be filled out and filed on line or you can print it out. I prefer to do it on line because you can request different things, even filing on line you must print out a signature page and mail it in. Good luck the form is a SF180.  (TC)  9/10/19
