Q235:  How can I receive and or review my medical records from and during Vietnam?  How do I request an audit on my records? I also need help in finding PTSD treatment documents placed in another veteran's file with the very same name.

Use Form Below To Respond

A1:  Go to your nearest Congressional office or VSO at a Service Organization (VFW, Am Legion, DAV) and submit a request to the Congressional office with a SF180. Ask for SF-88/89/93 SF-600 and all treatment records for your time in service. Then you and an Accredited VSO can review these records. You are looking for PTSD treatment records from where? If you went to Behavioral Medicine at a VA Health Facility. They have your records, if you went to a private facility they have your records. All claims are tracked by a file number or SS number with the VA. Could your files be with another vet with the same name, yes it has happened. An accredited VSO who can deal direct with the VA can assist you. A records audit can be done by you or an Accredited VSO.  (GH)  9/10/19
